It seems that even if you clear all missions and secondary conditions you cannot reach 100%. It appears that you only get credit for 94%.
Haven't confirmed later missions.

Hmm...sounds bad...ok, will look into it.
We did change something there, so almost certainly something we broke.

If he's referring to the mission in Act I. I couldn't achieve 100%, but I can't go back to screenshot now as I am on Act III :/

It should still show your percentage on the Act/Chapter select screen

If you were at 94-96% before (I forget what the exact number was) you should have 100% now, and if you had 100% on the other maps you should have the stamp for the first set of maps.
It should be fixed. If you still see 94/96% please let us know!
